2018 Community Health Needs Assessment Surveys

Health and wellness matters to everyone.
There are currently two surveys underway for community members to share their opinions and experiences concerning health issues. The surveys are similar and share a common purpose, but cover separate geographical areas. All responses are completely anonymous.
Individuals who live or go to school in Mercer County are invited to complete the surveys. The Greater Mercer Public Health Partnership, GMPHP, is a collaborative of healthcare facilities and local health departments in Mercer County and the assessment is specific to Mercer County. The Penn Medicine Princeton Health survey and assessment covers Middlesex, Mercer and Somerset Counties.
Please consider participating to help shape the outreach programs prioritized in Mercer County over the next few years. 
These surveys are just one of the tools used in development of the Community Health Assessments (CHA).  Data, focus groups and key informant interviews provide further insight into the needs of the community. Hospitals, public health agencies and non-profit agencies use the information collected to collaboratively strategize the focus of future programs aimed at health improvements.  This plan is known as the Community Health Improvement Plan (CHIP).
The CHA and CHIP process is ongoing and the reports are available to the public through the hospitals and public health agencies. The surveys can be found at the following links:
